Im 35 weeks pregnant i had a very important questions? please help!?

So ill be 36 weeks pregnant in 3 to 4 days. im working 6 hrs mon thru fri. I earn about 450 sometimes less sometimes more for maternity leave how much would they be giving me? also how many weeks do they give you off before and after baby? am i suppose to send my disability form already? please anyone that has gone through this or knows thank you!
